Banquet seating is limited to 120 registered attendees. Registration will close at the end of the day on Sunday, January 28, unless this event fills up earlier. No refunds will be issued unless the entire event is cancelled.

  • As long as the number of spaces remaining is greater than zero, your entire party will be confirmed.
  • If the number of spaces remaining is zero or less, your reservation will be waitlisted, and no payment will be taken at this time.
  • If your party is larger than the remaining spaces shown here, your entire party will still be confirmed.

How to Register for the Banquet

This form can be used to register just one MMS member, and also include other members and guests (non-members) to register and pay as a group. Non-members can only attend if they are included as guests by an MMS member who is registering.

Use the + row and – row buttons to add or remove fields for additional attendees, and then provide names for each attendee. Remove any unused rows or the total cost will not be correct.

Once you have completed the registration form, click the Submit button. If you selected the pay by check option, you’ll mail a check to the address in your confirmation email to pay. Otherwise you’ll be redirected to pay online with major credit cards or a PayPal account. All details needed for attending will be included in your confirmation email.

Registration summary (totals)

All attendees, including guests, must present proof of their full COVID-19 vaccination, including booster, at the door to be admitted. Masks are required to be worn inside, except when eating or drinking.

Payment

Check pay?
If you prefer not to pay online with a PayPal account or credit card, you can select the “Pay by check” option. This option will only be available during the first week registration is open. After you submit this form, promptly mail a check to the address provided in your registration confirmation email so it can be received a few days before the event.
